Forms and data collection

Forms that collect the data you actually need.

Build a form in three short steps, attach the documents people must accept, and share it by link or by email. Every answer lands in one list, and each one can become a signed document.

Three ways to use a form

Answers, a document, or a signed document.

Collect answers only

Answers are saved with each submission. Use it for registrations, surveys and data you only need to read.

Generate a document

Each submission’s answers fill a PDF from your template, such as an application or an order confirmation.

Generate and sign

Respondents sign the PDF right after submitting, and your company can sign after them. For contracts and NDAs.

Build it in three steps

From idea to shareable link.

01

Name and purpose

Give the form a name and choose what happens with the answers.

02

Document

Pick one of your templates, upload a PDF such as your existing contract, or start blank. Documentas can create the form’s fields from the template.

03

Respondents and signers

Choose whether respondents identify themselves, whether they can download their document, and who from your company also signs.

Field types

Every field you need.

Each field can be required, have a default value and a help text. Answers are checked as they’re entered, and mistakes show next to the field before anything is saved.

Text

Name, address, job title, comments.

Numbers

Whole numbers for guests or quantities, decimals for amounts, areas and weights.

Dates

Start date, date of birth, event date.

Checkboxes

“I have a valid licence”, “I agree to receive updates”.

Choices

Single or multiple choice, as a dropdown or checkboxes: plan, department, dietary needs.

Tables

Respondents add rows: order lines, family members, equipment, references, work history.

Read and accept

Proof that people read your terms.

  • Attach up to 10 PDFs, such as terms of service, a privacy notice or house rules
  • Respondents open and accept each one, in order, before they reach the questions
  • Every acceptance is recorded with its time and IP address
  • Write your own acceptance text, or keep the default: I have read and accept “Terms of service”

A form’s document settings: a review document to accept, the template that turns answers into a PDF, respondent signing, company counterparts and test documents
Table fields

Let people add as many rows as they need.

A table field has up to 10 columns, each with its own type: text, number, date, checkbox or choice. Respondents add up to 50 rows.

In a generated document the table becomes a real table, so order lines, family members or a list of returned equipment print the way you’d lay them out by hand.

Sharing

One link for everyone, or a personal invitation each.

One public link

Protected by a captcha. Turn it off at any time, or replace it with a new link so the old one stops working.

Personal invitations

Add people with their name, email and optional ID code. Their details are filled in and locked when they open the form. Up to 200 per form.

Answer limit

Cap the number of answers, for example 50 places at a workshop.

Open and close

A form is Draft, Active or Closed. Closing it stops new answers.

Named or anonymous

Identified respondents for anything tied to a person. Anonymous answers for surveys, feedback and reporting.

Test mode

Try the form end to end. Generated documents get a PREVIEW stamp and use no credits.

For the respondent

Easy on any phone.

Nothing to install and no account to create. Clear steps: identify, read and accept the documents, answer, review, submit, and sign if the form asks for it. Respondents can go back to earlier steps before they submit, and get a personal closing message.

Every answer in one place

See every answer, and how it got there.

  • One list of every submission, filtered by form and status and searchable by email
  • Statuses from Invited and In progress to Submitted, Signing and Signed
  • A timeline per submission: invited, email sent, started, documents accepted, answers saved, submitted, document generated, with times and IP addresses
  • Answers to fields you later removed are still shown, so nothing is lost when a form changes

Forms people actually finish

The form builder lets us create the form in minutes.

An award-winning coworking space in Brno gives every event its own registration link and reuses the form for the next one.

FAQ

Questions people ask

Can I collect data without signatures?
Yes. A form can collect answers only, generate a document, or generate a document and have it signed.
Can people add several rows, such as order lines or family members?
Yes. A table field lets respondents add up to 50 rows, and the generated document shows them as a table.
How do I stop spam or too many answers?
The public link has a captcha. You can set an answer limit, turn the link off or replace it with a new one. You can also invite people individually.
Can I send the form to specific people only?
Yes. Invite people by email with their name, email and optional ID code. Their details are filled in and locked when they open the form, and you see each person’s status. Up to 200 invitations per form.
Can respondents stay anonymous?
Yes. Turn identification off and the form collects answers only, with no personal details. Use it for surveys, feedback and anonymous reporting.
What happens if I change the form after people have answered?
Answers to fields you later removed are still shown on each submission, so nothing is lost.
Can respondents upload files or pay in the form?
Not today. Forms collect typed answers, choices, dates, numbers and tables. File uploads and payments are not part of Documentas forms.
