Consent collected, signed and on file.

Send each parent or client their own link. They read the terms, answer the questions and sign. You see who has responded and keep every signed consent.

Documentas
A respondent reviewing their answers and accepted terms before submitting a form
The problem

Paper slips that never come back.

Consent slips go home in bags and come back crumpled, or not at all. Clinic intake forms are filled in on a clipboard in the waiting room and typed up later.
